NEW APPOINTMENT INFORMATION:

​If you have an upcoming appointment or would like to review my information, please click on the "New Patient Forms" at the top right hand side of this page. You will see a drop down menu with several forms.  This may seem like a lot, and it is, but it will allow us to spend our time talking and coming up with a collaborative plan rather than filling out forms during our session.

The ones you will need to bring with you for your initial appointment include:

PTSD Screening Checklist *(this page has links to OCD, Panic Disorder, Generalized Anxiety Disorder, Social Phobias, and several others). Other links provided for ​General Anxiety Scale, Mood Disorder Questionnaire, and others screening instruments. You are welcome to complete any that you feel may apply to you and bring with you.

​If you do not have access to a printer or cannot access the links, we are happy to email or mail a hard copy to you.

Otherwise you should plan to arrive at least 30-45 minutes early. I don't recommend this as it is stressful to try and complete the paperwork immediately prior to your visit. I believe a comprehensive evaluation is important for optimal outcomes.


NOTE:On the intake form you will just complete until you see "STOP HERE". If unsure of the answers to any question it is fine to just leave blank and go to next item.

Communication

I make every attempt to have a streamlined communication process for appointments, medication management, and follow up visits for both meds and psychotherapy.  I cannot make non-emergent medication changes via text, email, FB messenger or other social media. Please do not ask me to do so. There are several problems in doing this. I do not have access to your chart and thus, cannot make a decision based on the best information, neither can I note in your chart the issues regarding requested changes. I will do everything possible to get you in for an appointment ASAP.


​I use an electronic prescribing system in order to assist my patients in not having to make an extra trip to my office to pick up a paper script and pay an extra visit fee. *Note if you are on any controlled substance it is a requirement that you be seen every three months. I utilize the Kasper system to monitor controlled substances as required by law and current regulations. If appropriate we will discuss this and you will be asked to sign a Controlled Substance Agreement Form. This is "standard of practice" in most all offices. ​​
